Get Your Amenity ID Card
SunCrest OA issues membership cards to all SunCrest residents who are current with their assessments and have no outstanding violations. The picture ID card will grant you access to the Community Center, including the fitness center, pool, and hot tubs.
Individuals 12 years of age or older are eligible to receive a card. The first card is free, and replacement cards are $10.00 each. Members must be 16 years or older to use the gym.

Please follow the instructions below to receive your Amenity ID card!
New Homeowners
Please fill out a Homeowner Information Sheet and email it to Daphne at dray@ccmcnet.com. If you have lived in your home for less than a month, please provide proof of home ownership with your Homeowner Information Sheet. Upon receipt of the form, we will input your family into the system, and you may obtain your membership card.
You may also bring the homeowner information sheet and proof of ownership to the SunCrest OA Office located upstairs in the clubhouse during business hours (M-F, 8 am-5 pm) if it is more convenient.
Renters
The homeowner must submit a Consent to Tenant form and a copy of the lease or rental agreement via email to Daphne at dray@ccmcnet.com before any tenant in the household is issued an amenity ID card.
This form states that the owner of the home grants their amenity access privileges solely to the person(s) leasing or renting the property. The property owner will not be able to use the amenities while granting use to their renters. Amenity access will be granted to renters for the duration of the lease agreement and will expire on the last day specified in the lease.
If renters choose to extend or renew their lease, please submit a copy of the new lease or rental agreement to Daphne at dray@ccmcnet.com to reinstate the renters' amenity access for the period specified in the new lease or rental agreement.
Once SunCrest OA receives the Consent to Tenant form and a copy of the lease or rental agreement, renters will be able to obtain membership cards.
You may also bring the Consent to Tenant form and a copy of the lease or rental agreement to the SunCrest OA Office located upstairs in the clubhouse during business hours (M-F, 8 am-5 pm) if it is more convenient.
Obtaining Your Membership Card
Please allow at least 1 business day after submitting your Homeowner Information sheet or Consent to Tenant form before coming to the clubhouse to obtain your amenity ID card. This allows us time to input your membership into our system.
After 1 business day, all residents aged 12 and above can visit the clubhouse to take a photo and receive a physical amenity ID card. Please bring your photo ID with you to receive your card. An adult in the household must accompany any child without a picture ID to receive their card. If no one is at the front desk downstairs, please visit the OA office upstairs.
All residents aged 18 or older must sign the SunCrest Clubhouse Liability Form when obtaining their card. Parents or Guardians must sign the SunCrest Clubhouse Liability Form on behalf of their minor children.
